What is the STAR Form?

STAR Form: Student Talk and Response Form

The STAR form is another way (in addition to talking to the safeguarding team, Head of Year or ANY member of staff) for children to be able to report any worries or concerns they may have in school or outside of school.

Please use this form responsibly. At Blaise High School our culture is only of support, respect and equality. Please make sure that you answer every question as this will help us to understand your needs/worries. Don't forget to press submit at the end.

Why do I have to give my name?

Just as in school, when worries or concerns are reported to a member of staff, the name of the student will be passed on to the Safeguarding Team. This means you or your friend can be supported correctly and quickly. If you are unsure whether something is a worry or concern, it is important to tell an adult, so it can be investigated. 

Who is the Safeguarding Team?

Where can I find the STAR Form?

Children will be able to access the STAR Form 24/7 on the Blaise High School website under: Students → Advice and Support → STAR Form. This can be accessed in school on school computers and at home on all internet-enabled devices.

When should I use the STAR Form?

When you see or hear about, in person or online:

  • Bullying (e.g ongoing, regular unkindness)

  • Friendship issues (e.g falling out with friends that you need help to manage)

  • Worried about a friend

  • Worried about a family member

  • Anxiety

  • Feeling unsafe

  • Discrimination/Racism

  • Sexual harassment/inappropriate behaviour (e.g touching, asking for photos, ignoring consent)
